A physical activity and diet program implemented by health educators working in a doctor's office may be a more effective way to get obese people to lose abdominal fat than advice from a doctor alone, according to a study from Queen's University.

Most primary care physicians do not have the time to provide high-intensity behavioral counseling to their patients, says the report by Robert Ross, a professor from the School of Kinesiology and Health Studies.

"The cornerstone of health care delivery is the doctor's office and the doctor doesn't have a lot of time to counsel and adequately monitor patients to get them to adopt healthy lifestyles. So the study placed a kinesiology-trained, health care professional in the doctors' office to see if they could produce better results and they did," says Dr. Ross.

The study looked at 490 sedentary, obese adults over two years. The patients who worked with a trained health educator within the primary care setting made major behavioral changes to their lifestyle over the long term resulting in significant reductions in abdominal obesity.

Dr. Ross says the findings are encouraging, but it would be premature to suggest the government should fund a health care professional in every doctor's office.

"The study provides promising resulst and it heads us in the right direction. We still have a lot to learn about how to get obese, sedentary individuals to adopt and sustain healthy behaviors over the long term," says Dr. Ross.

The report is published by Archives of Internal Medicine, one of the JAMA/Archives journals.

SEOUL, KOREA The International Vaccine Institute (IVI), an international organization based in Seoul devoted to research on the development and introduction of vaccines for the developing world, reached a milestone in its aim to prevent and control dysentery (also commonly called shigellosis or bloody diarrhea), a deadly disease that affects several million people worldwide. This infectious disease, caused by the bacterial pathogen Shigella, is a major health problem in developing countries, as young children are particularly vulnerable to the disease. The United States Patent and Trademark Office recently issued two patents on "Novel Shigella protein antigens and methods" to IVI. These two patents will accelerate the Institute's current efforts in developing an effective and low-cost vaccine for use among impoverished communities afflicted by dysentery.

The Shigella vaccine patents, which will expire in 2029, are the most recently issued patents of IVI. IVI filed these two patents (Applications No. 12/603,574 and No. 12/692,187) following its discovery of novel common protein antigens in several Shigella species that could be used as the basis of a universal vaccine against bacillary dysentery. With the potential to provide cross-protection across species and serotypes of Shigella, this could be the first broadly protective vaccine against dysentery. The patents claim vaccine compositions comprising either of two novel conserved Shigella proteins and a conjugate molecule comprising an O-specific polysaccharide antigen of Shigella bacteria covalently bound to either of these Shigella proteins.

Dr. Cecil Czerkinsky, IVI's Deputy Director-General of Laboratory Sciences and Dr. Dong Wook Kim, Associate Professor at Hanyang University and former IVI scientist, reported the original discovery of the Shigella common protein antigens in a provisional patent application filed in October 2008. Dr. Czerkinsky commented, "We are extremely excited about the issuance of the Shigella vaccine patents. It further reinforces our belief that these common proteins, through their immunological properties, may be highly effective in preventing shigellosis, a diarrheal disease that claims countless lives of children every year, mainly in developing countries."

OAK BROOK, Ill. ? Based on a study of nearly 2,000 breast cancer patients, researchers at the Swedish Cancer Institute in Seattle say that, in women between the ages of 40 and 49, breast cancers detected by mammography have a better prognosis. The study appears in the March issue of Radiology.

?In our study, women aged 40 to 49 whose breast cancer was detected by mammography were easier to treat and had less recurring disease and mortality, because their cancer was found at an earlier stage,? said Judith A. Malmgren, Ph.D., president of HealthStat Consulting, Inc.

Dr. Malmgren and a team of researchers reviewed breast cancer patient data from a dedicated registry at the Swedish Cancer Institute?s community cancer center. The researchers analyzed data on 1,977 breast cancer patients between the ages of 40 and 49 who were treated between 1990 and 2008. Over the same period, patient- and physician-detected breast cancer declined from 73 percent of all cases in 1990 to 42 percent in 2008.

?The shift toward more mammography-detected breast cancer cases was accompanied by a shift toward diagnosis at an earlier stage of disease that required less treatment,? Dr. Malmgren said.

Over the 18-year period, the number of breast cancers diagnosed at Stage 0 increased by 66 percent, while the number of Stage III breast cancers decreased by 66 percent. The majority of Stage 0 cancer cases were ductal carcinoma in situ (DCIS), a non-invasive cancer that is confined to a milk duct. The treatment of DCIS remains controversial, because not all experts agree that it is potentially life threatening.

Dr. Malmgren said another key finding of the study was the extent of treatment patients received. Compared with women whose cancer was self-detected or discovered by a physician, patients whose cancer was detected using mammography were more likely to have breast-conserving treatment and less likely to have chemotherapy. Specifically, they were more likely to undergo lumpectomy (67 percent versus 48 percent), less likely to undergo modified radical mastectomy (25 percent versus 47 percent), and less likely to die of breast cancer (4 percent versus 11 percent).

?The benefits of breast cancer treatment are accompanied by significant harms,? Dr. Malmgren said. ?Chemotherapy may have long-lasting toxic effects on a woman?s body, and mastectomy and reconstructive surgery are difficult and expensive operations that can have a significant effect on body image.?

The American Cancer Society and other medical organizations recommend that women should begin receiving annual mammograms to screen for breast cancer beginning at age 40. In 2009, the U.S. Preventive Services Task Force issued controversial new guidelines that eliminated the longstanding recommendation for routine mammography screening for women between the ages of 40 and 49, stating that the benefits of screening were potentially outweighed by the harms of screening, such as false-positive results.
